SDMS Membership Card

Full Name: If your name changes and you would like an updated card, please contact SDMS at membershipdept@sdms.org (be sure to provide your SDMS # and new name).

SDMS #: Your 6-digit SDMS membership number. Be sure to use the leading zero(s) whenever using your SDMS number.

Membership Category: Your current membership category. If you change SDMS categories (i.e., move from Active [ACT] to APS®), a new card will be issued. For more information on the SDMS membership categories, click here. SDMS membership cards cannot be used to verify Registry certification and should not be given to employers or others. If your employer wants verification of registry certifications, you must use the card/certificate provided by the Registry. Valid Dates: Your SDMS membership expires on the last day of the month listed. You will receive a renewal notice prior to this expiration date.

Member Since: Lists the year you joined SDMS. If your membership lapses and you later rejoin SDMS, your new join date will be listed.
